Most of these insurance plans require special forms or membership cards to be shown when you visit Dr. Easton's office. When making an appointment, please tell the appointment coordinator what type of insurance plan you have so your claim may be processed efficiently.

After you make an appointment with the office, or if you have a change in address, insurance or name information, please fill update the patient registration form and fax to 954-564-3869, or mail it to 1560 East Oakland Park Blvd, Oakland Park, FL 33334-4425, or bring it in with you to your next appointment.
